Name the visible symptom without explaining it yet.
Attention, Alignment, Authority, Adaptability, or another testable condition.
Which ceremony changes a decision? Where does feedback change the plan? Which control exists only because authority is unclear?
Name the decision, boundary, incentive, routine, or ownership gap.
Remove or combine one ceremony that produces no decision, evidence, or learning.
Use one accountable role or person, not a committee.
Lead time improves while quality, risk visibility, and stakeholder confidence remain stable or improve.
Choose a date close enough to learn, not merely to report.
4 · Measure the condition
Test the diagnosis before expanding the solution.
Distinguish useful integration from control with additional ceremonies. Use the assessment to add a structured signal, then compare it with the evidence in your Builder Brief.
