Name the visible symptom without explaining it yet.

What did the board approve that has not happened? Who owns the exposure now? What evidence would let directors challenge without operating?

Name the decision, boundary, incentive, routine, or ownership gap.

Replace one activity-heavy board update with an outcome, owner, exposure, trend, and decision request.

Use one accountable role or person, not a committee.

Directors can identify the gap, accountable owner, and required decision from one page.

Choose a date close enough to learn, not merely to report.

4 · Measure the condition

Test the diagnosis before expanding the solution.
