Name the visible symptom without explaining it yet.

What are we trying to make true? Which assumption has not been tested? What would prove we understood the decision differently?
Name the decision, boundary, incentive, routine, or ownership gap.
End one decision with written intent, owner, tradeoff, and next evidence, then compare interpretations.
Use one accountable role or person, not a committee.
The team executes the decision without preventable rework from competing interpretations.
Choose a date close enough to learn, not merely to report.
