The team had the documents for years. What it did not have, until somebody with standing said so, was permission to be looking at the system rather than the case.
What happens
A newspaper has covered a series of individual cases for years, one story at a time, each one closed when it ran.
A new editor arrives, reads the coverage, and redirects the investigative team from the individual to the institution that kept moving him.
The reading
The reallocation is the whole intervention and it takes one sentence. The facts do not change and the team does not change. What changes is the unit of analysis the organization is paying attention to.
Notice what it costs. The team produces nothing for months while the rest of the paper ships daily, and the editor absorbs that pressure rather than passing it down as a request for interim output.
The team also stops chasing the story that would run soonest. Holding for the pattern rather than publishing the first case is an attention decision made repeatedly, not once.

What would have changed it
Give the same reporters the same documents with the old unit of analysis and the work is competent and useless. Each case is reported, each case is closed, and the structure that produced them is never in view.
The failure mode is not laziness. It is that a series of correct small answers can occupy an organization completely and leave the question that matters unasked.
Where you have seen this
Your team closes tickets well. Nobody is asking what keeps generating them, because that question does not fit inside a ticket.
What to do about it
A reading that ends in understanding is entertainment. This is filed under Training, so it ends in things a team can actually do.
Take one recurring problem and forbid the team from reporting instances of it for a month. Ask what produces it instead. The instances will still be there afterward.
When you protect a team's attention, absorb the cost yourself rather than asking for interim deliverables. An interim deliverable is the old unit of analysis coming back through the side door.
Say the reallocation out loud and say what it displaces. Attention given to something new is attention taken from something, and leaving that half unsaid is how the new work quietly gets added on top.

Issue selling, and the scarcity of executive attention
Attention at the top of an organization is a scarce resource that is allocated rather than discovered, and the literature on issue selling treats getting an issue onto that agenda as an activity with its own moves and its own risks, performed by people below the top. The attention-based work distinguishes the perspective a decision maker brings, the engagement they give a particular issue, and what is finally selected, which is why a reallocation can change outcomes without any new information arriving.

What this is not
