Where has a technically successful pilot stalled? Which decision or ownership boundary blocks production? What work will stop or change if the capability scales?

Name the decision, boundary, incentive, routine, or ownership gap.

Remove one operating-model constraint from the strongest pilot before expanding its technology footprint.

Use one accountable role or person, not a committee.

The pilot moves one stage closer to a customer-visible or operating outcome without adding approval layers.

Choose a date close enough to learn, not merely to report.

Test the diagnosis before expanding the solution.
